Team Application (Team Reservation Form)

*To be completed by Team Leaders (i.e. Youth Pastor)

Step 1: Register Your Team and complete the necessary forms. If you are new to the online application system, you can create an account using the link above.

Step 2: Mail in Deposit Form and deposit ($200) to the Central Canada Office before February 1, 2013.

Step 3: Add Team Members under the Team Control Panel. Each team member will receive an email outlining their next steps.

Step 4: Ensure team members complete the below Application Process and submit their ministry support ($450) before March 1, 2013. 

Step 5: Once we have received your Team Reservation Form and deposit, we will process your application.  Acceptance/Training Manuals are mailed within two weeks of receiving your information.

    2. *To be completed by members registered with a team. Application can only be completed after you receive an email from your team leader with an activation link.
    1. Step 1: Activate your Account (an activation email will be sent to you by your team leader).

    Step 2: Register as a Team Member and complete the necessary forms.

    Step 3: Go to Pre-Program Training and complete the Mentor Covenant and mail the form to the Central Canada Office.

    Step 4: Submit your ministry support ($450) and paper application forms to the Central Canada Office before March 1, 2013.

    Step 5: Once we have received your forms, we will process your application.
